Humana Inc. is partnering with Boston-based Iora Health to open two physician offices catering exclusively to Humana’s Medicare Advantage enrollees.

This new accountable care arrangement will launch four primary care practices later this month — two in Phoenix and two in Seattle.

The goal is to use the practices as models to open others in Arizona and nationwide, said Rick Beavin, Humana’s Arizona market president.

Humana has about 55,000 Medicare Advantage enrollees in Arizona.

The partnership includes incentives tied to performance improvement and will bring better patient care coordination and more cost-effective care, he said.

Plans call for offering more personalized care and a range of population management tools, such as predictive analytics, chronic care, disease management and wellness programs.

Iora, which serves more than 9,000 patients in seven primary care practices in four states, is making its Arizona debut with this Humana partnership. The practice integrates health and wellness and behavioral health services. It also offers groups for those with diabetes as well as yoga clubs.

The Arizona primary care practices are at 3137 W. Indian School Road in Phoenix and 6611 W. Peoria Ave. in Glendale.
